Medication Authorization Form for Dispensing Medications At School
To comply with NYS medication guidelines, school nurses are not permitted to dispense any medication to students without an order from the child’s physician. This includes all over the counter medications. Please fill out the above form to give the nurse authorization to dispense medication to your child.
